Create New TRF

If you want to create a new TRF, please click the plus icon.

Submitted Test Requisition Forms;

  • From this section you can see all the Test Requisition Forms you have sent to us.
  • The system will automatically tag the form as “Form Submitted”. The status will change as the process change.

Progress Indicator; Submitted (Indicates a newly created TRF entry.)

Progress Indicator; Progressing (Indicates that the TRF you created has been processing.)

Progress Indicator; Rejected (That you could not reach the next step in the TRF entry you created as a result of any missing or incorrect information.)

Progress Indicator; Verified (It indicates that you have successfully completed the whole process and that your certificate is ready.)

You can export all your TRF entries as "CSV Table".
